Homeless Health Care Navigator – Westminster

Reporting to: Westminster Homeless Health Care Navigator
Hours: Full time (37.5 hours per week)
Contract length: Initially until March 2021 but continuation funding has been applied for
Salary and Benefits: £23,500 plus 5% pension and 28 days annual leave (plus bank holidays)

Groundswell is seeking a worker with lived experience of homelessness, substance abuse or living with mental illness for a Public Health England funded Homeless Health Care Navigator role.  This is an opportunity to develop and deliver an important new area of work bringing together key agencies to ensure the health needs of vulnerable homeless patients are being met. You will be playing a crucial role in a growing and dynamic charity which is committed to developing inclusive solutions to tackling homelessness.

The role is based in Westminster, working alongside a multi-agency team of specialist homeless nurses, psychologists and drug and alcohol workers.  The team will identify people experiencing homelessness, who are also struggling with issues around mental health and substance misuse and work together to provide wrap around support around all aspects of someone’s health. The Homeless Health Care Navigator will support the people they work with to access appropriate healthcare and work towards managing their own health.
