How to refer

1. Please check the table below whether HHPA is available in your London borough

2. If so, please use our form below to either:

  • refer an individual experiencing homelessness to Groundswell’s HHPA service
  • request support for an upcoming appointment for an individual already registered in HHPA service.

If you want to refer someone for HHPA service, please fill in the form below.

If you need support to attend a health appointment for yourself, please send us your details using the Self Referral Form we will contact you to discuss how we can help.

Homeless Health Peer Advocacy (HHPA) began in Westminster in 2010.

It has since expanded to a wide range of London boroughs.

In the 10 years HHPA has been operating in London over 130 individuals have completed training to become volunteer Peer Advocates, delivering more than 23,500 one-to-one appointments and group health promotion sessions.
